A “distribution” of your account means that you want to rollover your funds into another qualified 401(k) or IRA account because one of the following circumstances has occurred:

You have two choices for completing the BCN Services distribution form:

Online submission - Log in to your Slavic 401(k) account to complete the form online and submit electronically with a digital signature. Online form submission cuts the processing time in half.

Print Form - Print your form to sign and send in to us.

In order to access these or any other Adobe Acrobat file available online, you will need to Get Acrobat if it is not yet installed on your computer’s hard drive.
